Use the ER Diagram, Relational Schema and Meta data that you created for the

Clinic ER Modeling Assignment.

Write the CREATE TABLE Statements for the tables in your submitted relational schema.

Create some fictitious sample data to populate instances in your tables.  5-10 instances per table.  

Write the INSET INTO Statements that will insert your fictitious sample data into the tables that were created with your CREATE TABLE Statements.

Your script should contain a CREATE DATABASE and USE statement at the beginning of the script

Your CREATE TABLE statements should be listed in the proper referential integrity order

Your INSERT INTO statements should be listed in the proper referential integrity order.
