For this assignment, examine the following scenarios in a clinical setting; Patients are treated in wards in the clinic, the patients are seen by doctors, doctors have assistants, patients may also be seen by the doctor’s assistants, assistants are assigned to wards. Create an Entity-Attribute-Relationship Diagram to represent the conceptual model of a database that could connect the transactions in these scenarios. Create any entities that would be needed, create any associated relationships. Identify the primary keys and the foreign keys that you would use. Finally include a couple of descriptive attributes for any Entity or Relationship items in the diagram that you think are help identify them. Remember for this course, we are only concerned with the (EAR) segment of modeling. Identify the Entities, their Attributes, the Relationships associated with the each entity. Connect these items using a connection line. We also discussed cardinality; try to identify the cardinality in the relationships. This project may be completed in Visio, Word, or another modeling software application. In the event you are limited to access to a computer due to the inclement weather, write the diagram on paper at home and transfer to your modeling tool of choice when you get the opportunity.
After creating a version of the diagram, translate your version of the ER Diagram into a relational schema.
Using your version of the ER diagram and your relational schema created from the diagram, create the metadata/data dictionary for your conceptual design of this database.
